The New York State Department of Health (NYSDOH) AIDS Institute and Health Research, Inc. (HRI) announce the availability of $2,320,000 in state and federal funds for : Component A: Enhance the capacity for the medical management and treatment of persons infected with the hepatitis C virus (HCV) in primary care settings in NYS. Component B: Enhance the capacity for the medical management and treatment of persons coinfected with HCV and HIV.Component C: Develop and implement an HIV engagement and retention in care program for African Americans/Blacks and Latinos receiving or eligible for primary health care services in an organization located in one of the eligible regions of Rochester/Finger Lakes, Buffalo/Western New York, or the Mid-Hudson counties of Sullivan, Ulster, and Orange.

Funds under this solicitation are intended to supplement, enhance or expand, not supplant, existing resources and services. Sources of support for this RFA are subject to change but at this time include $1,060,000 in funds from Ryan White HIV/AIDS Treatment Modernization Act; and $1,260,000 in funds appropriated from New York State. Individual award amounts will vary depending on component, number of program sites, size of the population to be reached, intensity of activities to be conducted, services to be provided, availability of similar resources, and agency capacity. For each component, funds are awarded regionally. |

| Applications for all components of this RFA must be received by the NYSDOH/HRI at the following address by 5:00 PM on December 8, 2009. Late applications will not be accepted. Send the application to Valerie J. White, Deputy Director, Administration and Data Systems, New York State Department of Health AIDS Institute, ESP, Corning Tower Room 478, Albany, New York 12237. A separate application is required for each submission in response to Component A, B and C. Applicants shall submit 1 original and 4 copies, including all attachments. The original should be clearly identified and bear the original signature of the Executive Director or Chief Executive Officer (Attachment 5 of RFA at http://www.health.state.ny.us/funding/rfa/0812241006/0812241006.pdf) and Director of Medical Services (Attachment 6 at RFA http://www.health.state.ny.us/funding/rfa/0812241006/0812241006.pdf) of the organization submitting the application. Application packages should be clearly labeled with the name and number of the RFA as listed on the cover of this RFA document. Applications WILL NOT be accepted via fax or e-mail. Applicants should pay close attention to Attachment 2, the Application Checklist, to ensure that submission requirements have been met. Applicants should review this attachment before writing and prior to submitting the application. It is the applicant’s responsibility to see that applications are delivered to the address above prior to the date and time specified. Late applications due to a documentable delay by the carrier may be considered at HRI/Department of Health's discretion. |
